Kenzaburo Tenma, a Japanese surgeon working at the Eslar Memorial Hospital in West Germany, is known as the "genius doctor." His surgical skills are exceptional, and he is humble and well-respected by both colleagues and patients. His career is bright, and he is highly regarded by the hospital director. In his personal life, he is deeply in love with Eva, the director's daughter, and their relationship is as strong as ever. However, despite his seemingly idyllic life, Tenma begins to feel a growing sense of moral unease. He witnesses the hospital prioritizing the rescue of important figures over ordinary patients in critical condition, and the accusations and cries of the bereaved families weigh heavily on his conscience. When faced with another such moral dilemma, Tenma resolutely chooses to save a boy with a head wound. He sacrifices his career and love, but in return, a terrifying monster is revived. In the years that follow, Tenma and the monster John engage in a game of wits and courage, as a buried history gradually comes to light... This film is adapted from the eponymous original work by Japanese manga artist Naoki Urasawa.
